Ultrasonographic Evaluation of Nipple Discharge

Abstract
Purpose: To evaluate whether breast ultrasonography is helpful in distinguishing benign from malignant lesions and clinical features associated with malignant histology in women with pathological nipple discharge.

Materials and Methods: Between March 2004 and August 2010, breast imaging report database search revealed 1533 consecutive women with nipple discharge in our institution. Patients with known breast cancer (n=127), patients who had undergone mammography alone (n=155), patients without one or more imaging follow-up record (n=595) were excluded. Finally, imaging reports and histology reports of 656 women with nipple discharge were analyzed. Rate of malignancy according to the final assessment category on breast US and clinical features (palpability, color of discharge) associated with malignant histology were analyzed.

Results: Of the 656 patients, 13.4% (88) were malignant. The rate of malignancy according to the final assessment category was as follows: 1% (1/105) of category 1, 3% (2/60) of category 2, 2% (2/105) of category 3, 16% (58/361) of category 4, and 100% (25/25) of category 5. The sole clinical feature associated with malignant histology was a palpable mass.

Conclusion: Although palpable symptom and single duct discharge clinical features associated with malignant histology, breast US evaluation can help distinguish benign from malignant histology, and thus reduce surgical excision in patients with final assessment category 1 to 3.
